Would need a really nice looking car (or for humour, a clapped out banger).

The boyfriend would have his back to the camera, and the girl or guy whose secret it is, would be hugging him, but holding the car key with one hand, and pointing at it with the other. A cheeky wink wouldn't go amiss.
